About the Proof of Debt Form

A Proof of Debt (POD) form is the official document used for validating your claim. This form, along with supporting documents, serves as evidence for the Liquidator to verify your claim against the Company, and any potential entitlement to asset distributions.

Submission of a POD form is mandatory for your claim to be considered. Without a valid submission:

  • Your claim may not be recognised by the Liquidator.
  • You may forfeit entitlement to any potential asset distributions.

How to Submit Your Claim

Please follow the steps below to submit your claim using the online “Submit a Claim” portal:

Step 1: Download the official POD form using the link provided. 

Step 2: Complete all required sections of the POD form.  

Step 3: Sign and Date the form.(*) 

Step 4: Fill out the digital “Submit a Claim” online form below.(**)

Step 5: Attach the POD form and supporting documents with your electronic submission.(***) 

To assist creditors, a sample Proof of Debt form has been made available above for reference.

* Unsigned form will be rejected

** Details in the digital claim form (Step 4) MUST EXACTLY MATCH your signed POD form. Discrepancies may delay the validation of your submission.

*** Please note that copies of all documents supporting your claim must be enclosed with the completed Proof of Debt form, failing which, your claim may be rejected.

By submitting this claim, I declare that

(a) To the best of my knowledge and belief, the Company owes the amount claimed;

(b) I am duly authorized by the creditor (or under the creditor company’s seal) to complete this proof of debt form; and

(c) I have uploaded all supporting documents substantiating my claim. I acknowledge that claims submitted without supporting document will be automatically rejected.
